I once sat in a retrospective for a project I was the tech lead for. Everyone had positive praise for something I thought was very basic. When I had a problem to address, I got up from my chair, walked across the office to the other room, and spoke directly to the person who could help me. Everyone thought that was revolutionary, because the culture at that company forbade that cross-discipline style of working. The client was happy too, with a working system where we caught problems early.

The Managing Director wasn't happy overall though, because the project was about a week or two over budget and I didn't follow his rigid working process - that each department works separately and in order. I didn't work there for much longer after that.
